Liaison Nurse

Are you a nurse looking for a more office-based role, to take a step back from direct patient care, or a daytime position with a flexible weekday schedule? Excel Healthcare are delighted to partner with our client who are looking to hire a Liaison Nurse for their 100+ bed, residential care facility in South Dublin.

The Liaison Nurse will be responsible for the co-ordination of the transitional care unit, working closely with the major hospitals in Dublin. The job involves processing referrals, assessing appropriate admissions, and facilitating discharges to the community or to long-term care. The successful candidate will also need to communicate regularly with residents, their families, and the multi-disciplinary team, to ensure a smooth transition for the residents.

The successful candidate will be a charismatic nurse with excellent interpersonal and organisational skills, ideally with hospital or residential care experience.

Responsibilities of this Liaison Nurse job include:

  • Working closely with the HSE, residents, and their families
  • Responsible for the assessment of care needs
  • Management of logistics of admission & discharge process
  • Co-ordinating bed availability for admissions and discharges
  • Arranging and attending scheduled formal reviews with the MDT of individual residents’ care plans
  • Liaising with pharmacy regarding patient’s prescriptions

Requirements of this job

  • A charismatic personality with the ability to maintain excellent relationships with all stakeholders
  • Some acute nursing experience would be a distinct advantage
  • Excellent organisational and administrative skills
  • Active NMBI Registration
