We would like to invite applications for the post of Registrar in General Medicine / Ambulatory Care for the Intergrated Care Program for Older Persons Team /Kerry Residential Care Community Support Team.

Purpose of the post:

  • To be part of a greater multidisciplinary team providing emergency and ambulatory care for older patients living in their own homes or in the residential care facilities in Kerry
  • Provide Triage support of referrals from the Residential Care Facility Community Support team
  • To intervene early with Comprehensive Geriatric Assessment (CGA) to identify & reduce the risk of adverse outcomes
  • To create a truly integrated countywide, community led service that reduces unnecessary ED attendances, subsequent hospital admission and provide an alternative assessment route for ill frail older patients.
  • Proved medical support to the Ambulatory Care Hub in Kerry and a key member of the MDT providing specialist Geriatric care in the community
  • To support the  transfer of care to the most appropriate setting to ensure ethos of right care, right place, right time
  • To have a positive impact on patient experience in line with the goals and recommendations set out by Sláintecare, ICPCOP and Health Ireland initiatives
  • To increase the collaboration with third sector services e.g. ALONE, Age Action, SAGE
  • To align with local developments and national policy e.g. @QIImprovers, Five Fundamentals, ICPOP, Slaintecare
  • To develop a care plan and actions to align with #whatmatterstoyou and support hospital movements to #getupgetdressedgetmoving
  • To standardise work practices to reduce variation between acute care and community services
  • To collaborate with our academic partners and be a critical link in the evaluation of service and service development

Eligibility Criteria, Qualifications and Experience:

  • Applicants are expected to have completed Basic Specialty Training in General Medicine
  • Have minimum 12 months experience at Registrar level in General/Geriatric Medicine
  • Possession of  MRCP
  • Candidates are expected to be ACLS providers. Core requirements for this post include: a demonstrable commitment to medicine, good decision making, a broad clinical knowledge base and appropriate clinical skills, awareness of patient safety and risk management issues and the ability to work efficiently under pressure and as part of a team.
  • Each successful candidate must hold General Registration or Trainee Specialist Division Registration with the Irish Medical Council.
